Everything on this site is general information about Anthem Dental Group and the care we provide. It is not a diagnosis, not a treatment plan, and not a substitute for being seen. Nothing here creates a dentist–patient relationship — that begins at your first appointment.

In an emergency

Do not use this website, the appointment form, or the chat assistant to report a dental or medical emergency. Call your nearest office directly, or call 911 for a medical emergency. See emergency dentistry for what we can do and how quickly.

Appointment requests

Submitting the request form asks us for an appointment — it does not book one. Your appointment is confirmed only when our team contacts you. Please don’t send medical or health details through the form; see our privacy policy.

Prices and offers

Any price shown on this site applies as described on the page where it appears and may change. The $125 New Patient Special is for new patients only. Insurance coverage, benefits and any out-of-pocket amount depend on your individual plan and are confirmed before treatment begins.

Reviews and third-party content

Patient reviews shown on this site are published as written by the patient and are synced from Google Business Profile. Embedded maps and review data are provided by Google and are subject to Google’s own terms and privacy policy.
